Failure to wire properly or maintain electrical equipment can lead to death especially in petrochemical exposures. Training electrical maintenance folks with quality arc flash training and electrical safety training using NFPA 70E and giving regular training in proper electrical maintenance standards and techniques from standards like NFPA 70B and NETA MTS 2007 go a long way to preventing these types of injuries and fines. Often electrical hazards lead to the ignition of combustible dust or in this case a flammable liquid, naptha.
