As we point out. Just about every major fine includes fines for electrical safety. Seems like electrical safety is the canary in the coal mine for safety. It is usually technical. If you don’t have a technical safety team, you will not catch electrical hazards and probably others. As more companies use NFPA 70E for electrical safety, use ANSI Z10 for a continuous improvement safety process and have electrical safety audits, these issues start getting handled.
