This case seems a good example. The policy didn’t even cite the right laws. Lots of companies put things in writing the workers never even hear about. No training, no auditing, and no real knowledgable supervision of arc flash and electrical safety hazards. This company “required” all work to be done de-energized in writing but it never happened in practice and they had no electrical safety training.

Sad to hear when companies have policies that really don’t get into effect…
